Position Description:

The School-Age Service and Support Administrator (SA SSA) is an operational position within the Perry County Board of Developmental Disabilities (PCBDD), meaning its primary role is to carry out daily responsibilities to create the highest level of efficiency possible to meet the expectations and needs of customers. Under general guidance and direction of the Children’s Program Director, the responsibilities and expectations of the SA SSA are to serve as the primary point of service coordination for children and families who are eligible to receive PCBDD services and supports. The SA SSA is responsible for facilitating and connecting people with developmental disabilities to person centered services and supports.

Duties:

60% Planning & Coordinating

• Serve as single point of service coordination for children aged six to fourteen (unless the child is on a waiver) with developmental disabilities.
• Maintain a caseload of children that requires establishing and maintaining contact with children and families to identify their concerns and priorities and to acquiring the services and supports they choose for themselves within the diverse resources available.
• Provide service coordination and assure the planning process includes but may not be limited to:
  • Lead and facilitate planning by developing the Family Plan for the School Age Service Plan.
  • Identify person-centered services and supports to ensure education and training are implemented consistently with the child’s identified needs.
  • Connect the child and family to resources and supports identified during the planning process.
  • Facilitate relationships and partnerships with identified resources.
  • Monitor the implementation of ongoing services to ensure the child’s health and welfare.
  • Work in collaboration with other resources, customers and partners to resolve conflict.
  • Attend team meetings including but not limited to Individual Education Planning (IEP), Ohio Rise, Family and Children First (FCF) Service Coordination, and transition planning.

20% Academy for Leadership Abilities® and Summer Camp

• Participate in the Academy for Leadership Abilities® (ALA) by attending team meetings and serving as ALA Facilitator, as needed.
• Maintain case notes, documentation, and ALA Calendar updates.
• Serve as lead staff on assigned summer camp days.
• Obtain and maintain medication administration certification.

15% Case Notes and Documentation

• Maintain case notes, evaluate documentation of other providers, and ensure services are provided as specified in each child's/family's plan.
• Assist children and families in evaluating the effectiveness of the services and supports provided and adjust, as appropriate.
• Maintain necessary records and reports in a timely and accurate manner consistent with agency and regulatory standards.
• Incorporate the results of evaluations, ETR, IEP, consultations, and behavioral support services in the Family Plan.
• Identify trends and patterns of incidents in the plan to improve and enhance the quality and appropriateness of services rendered to the child.

5% Miscellaneous

• Participate in training and education to promote continued learning and professional growth.
• Perform other duties as assigned by director or superintendent.

Requirements:

Successful candidates must meet these requirements; reasonable accommodations may be made to enable people with disabilities to perform the essential functions.

• Bachelor's degree in education, psychology, social work, or related field and/or eligible for SSA certification (or grand-fathered) per Ohio Administrative Code (OAC) rule; associate degree with experience may be considered at the discretion of the Superintendent
• Strong organizational and time management skills; excellent interviewing and documentation skills
• Strong computer and technology experience including the use of Microsoft Office 365, web-based applications, and various technology systems; experience troubleshooting basic technology-related issues
• Valid driver’s license with acceptable driving abstract to meet criteria for insurability, driving is an essential function of the position.
• Maintain confidentiality, handle sensitive data, and comply with all applicable laws and Board policy regarding confidential information.
